Digital health and behavioral-health scale-ups face a specific squeeze:
- Compliance is non-negotiable. HIPAA, SOC 2, state-by-state behavioral health rules. One slipped audit and the company is paying lawyers instead of clinicians.
- The team is too small to own it. You have engineers shipping features. You don't have someone whose job is to keep the platform credentialed and resilient.
- AI is now table-stakes. Patients expect agentic intake; clinicians expect AI-assisted documentation. Most of your team has never deployed an LLM under HIPAA constraints.
- A full-time CTO costs $300k+ all-in. Your stage doesn't justify it. But "no CTO" doesn't work either.
What this is
A monthly retainer engagement. 5–10 hours per month, depending on what's actually in your queue. Three-month minimum, then month-to-month. Typical month includes:
- Compliance posture review — what's drifted, what needs to be fixed, what's coming up.
- Architecture review — the design decisions in your queue this month; what tradeoffs to make.
- Vendor and stack review — sanity-checks on the tools your team is considering or replacing.
- Agent governance — if you're shipping AI features, what guardrails go in, what evals get run, what doesn't ship yet.
- Hiring support — JD review, technical interviews for senior hires, leveling calls.
- Board / investor prep — technical narrative for the next deck or update.
What you get
- A senior practitioner on call — typically Slack + monthly leadership review + 1–2 working sessions.
- A compliance baseline if you don't have one yet (HIPAA, SOC 2 prep, audit-ready posture).
